Dr. Jon’s Exclusive Dental Membership Program

(Exclusively created for patients without dental insurance and cannot be used in conjunction with any dental insurance plans)

$325 – Annual Enrollment Fee
$225 – Each Additional Family Member

Benefits Include:

  • Initial New patient exam, One routine cleaning or Periodontal maintenance and X-Rays (A $400-500 value)
  • $50 co-payment for the second routine Cleanings Per Year or $100 for periodontal maintenance (deep cleaning not included)
  • Two Exams (includes emergency/problem focused exams)
  • One Set of check-up series X-Rays
  • 15% Reduced Fees For General Restorative Treatment (deep cleanings included)
  • Invisalign, TMD/TMJ Treatment, and Complex Full Mouth Reconstruction Cases Not Included in Program
  • Teeth whitening (Opalescence Go 4 trays)

Insurance

We are happy to work with your dental insurance. After you receive services from our office, we will bill your insurance company on your behalf. If there are any questions regarding your dental benefits, our experienced team is ready to assist you in any way.

We know that dealing with insurance companies can be very challenging for both our patients and for us. In order to help with this process, and limit the “unknowns” to our patients, we provide a best estimate of your responsibility for the procedure(s).
